Phase 3 of the Lighthouse Hospital Project was announced by the Hon Ken Wyatt AM MP, Minister for Indigenous Health on 10th February 2017. 

 

Phase 3: 2017 – 2019

Aim:

•          To reduce the incidence and impact of Discharge against medical advice (DAMA) for Aboriginal and Torres Strait Islander people who experience coronary conditions by supporting hospitals to ensure they provide care that is evidence based, responsive and accessible, clinically appropriate and culturally safe

•          To enhance relationships and coordination of care between hospitals, local Aboriginal and Torres Strait Islander Community Controlled Health Organisations (ACCHOs), and community health and Primary Health Networks.

•          AHHA and the Heart Foundation

•          18 hospitals across Australia that represent nearly 50% of all Aboriginal and Torres Strait Islander admissions for ACS

What activities will be undertaken:

•          Initial activities are to provide training and support to each hospital site on skills to identify and implement quality improvement activities in their hospitals

•          AHHA and Heart Foundation staff will assist in developing engagement and networks within local communities between hospitals, Primary Health Networks, Aboriginal Community Controlled Health Organisation and cultural leaders in the community.
