  • Oregon Patient-Centred Primary Care Home Program describes its values and standards in patient-centric language—not organisational or provider focused terms; and its clinics are required to support 24 hour access to care.
  • Oregon Health Authority/Portland State University research found that the most important attribute for success in patient-centered medical homes is coordination and integration of care, including with acute care services. Formal logistical agreements with hospitals were required.
  • Based on evidence, Oregon health insurers must spend >12% on primary care compared with about 7% in Australia.
  • Discussion, led by Chris Carrera, on the Oregon Patient Centred Primary Care Home program with Oregon Health Authority. Enrolment of patients a key to success. Significant savings demonstrated, greater focus on screening and prevention.
  • Patient-Centred Primary Care homes in Oregon: for every $1 increase in Primary Care expenditure, there is $13 savings in specialty care, acute care (ED and inpatient). Savings in total cost of care per patient have doubled over a 3-year period.

  • What worked to help Mountain View Family Practice become a high performing patient-centered medical home? Relationships with community partners (specialists), provider leadership, practice redesign, and Lean workflow (see http://ahha.asn.au/lean-training).
  • Collaborative relationships between primary care and other providers (e.g. hospitals, specialists, allied health) and practice-based care teams, with task designation by skill set: part of the TransforMED Patient-Centered Model used at Mountain View practice.
  • Business-as-usual at Mountain View Family Practice, Hillboro, Oregon: team huddles; Lean processes; patient satisfaction surveys; teams including a quality improvement manager, medical scribes, medical assistants, physician assistants, family nurse practitioners, and a behavioural specialist.
