Designed to enhance consumer outcomes and experiences of care through supporting shared decision making, Patient Reported Measures (PRMs) can help inform improved models of care, referral pathways, and partnerships to strengthen the health system and drive value. Effectively driving improvements through PRMs relies on engagement and coordination at all levels of the system, at the point of care (micro), across the pathway (meso), and at the population level (macro). Yet, application is fragmented across our health system as the need for coordination, standardisation and integration across the various levels remains a challenge.
So how do we create the conditions to embed PRMs in practice and then use the information to inform improvements in care across all levels of the system?
In our next VBHC webinar we will explore the leading work of NSW Health in embedding PRMs in everyday clinical practice using the Health Outcomes and Patient Experience (HOPE) platform. We will hear from a panel of leading experts, about their experience of embedding PRMs in practice, the conditions necessary to drive change, and how the data is being used to understand need, support real time decision making and drive improvements at all levels of the system.
