Information: The purpose of health care is to improve the health outcomes of the person and family being served. This session shows an example of a team that improves outcomes by changing their care and its measurement of patients’ results. You will explore the power of shared aspirations to provide caring, relationship-centred services that achieve better results for patients and families, and thus high value—better outcomes for the money spent.

Webinar title: Creating value for individuals and families

Date: August 20th, 9:00 am - 10:30am 

Information: Health care value is created when an individual’s health outcomes improve. This session analyses an example of team-based care that offers a combination of medical and social services to create a more holistic solution, and through its health successes drives down overall costs of care. You will explore the importance of understanding both explicit and unarticulated needs to develop high-value solutions for care and in life outside of clinical settings.

Information: Measurement focuses on managerial attention, so it is critical to measure what matters most to patients and clinicians. This session will look at an example where measurement enabled significant improvement for patients as well as support for the professionalism of clinicians.  You will learn the Capability, Comfort, and Calm Framework™ to measure outcomes that matter to patients and build solutions that accelerate this change.

Date: October 22nd, 9:00 am - 10:30am

Information: Successful health care transformation depends on financial sustainability. This session will look at the impact of different approaches and assumptions about measuring costs and how these may change managerial decisions or priorities. You will explore the relationship between care delivery and cost, and how to use this information for strategic decision-making.

Information: Leading change in health care depends on multidisciplinary teams that leverage strong relationships and effective communication to achieve their goals. You will learn how to build effective teams and to lead from wherever you sit within the health system.
