Primary Health Study Tour: United States April - May 2018

Date: 25 April 2018 9:00am to 03 May 2018 05:00pm

Learn more about patient-centred medical homes

Invited: Primary health leaders, policy makers and funders

25 April - 3 May 2018
Portland, Oregon, San Francisco and San Diego, California

Following on from the success of the US Study Tour for Health Executives in October 2016, AHHA is again inviting primary health leaders to join us in exploring the progress achieved in the United States in developing patient-centred medical homes. Australia adopted a trial of health care homes in July 2017, but more needs to be done to inform the rollout of this initiative. Study the US approach by joining an AHHA tour to the Institute for Healthcare Improvement Annual Primary Care Summit, with visits and master classes focused on best practice initiatives led by world leaders in health care.

Jack Cochran MD, former Executive Director of the Permanente Federation at Kaiser Permanente, will join the tour to lead masterclasses and guided reflective learning. Dr Cochran is a strong advocate for integrated care and physician leadership, while always maintaining a focus on the patient, even outside of the clinical setting. Dr Cochran urges all those with whom he works to bring the patient perspective into each conversation and decision. He invites physicians to join in catalysing health care reform by embracing their roles as healers, leaders and partners.
