The Australian Centre for Value-Based Health Care is pleased to present the next in our ongoing series of free webinars discussing the HealthLinks: Chronic Care program. Tanya Sewards, Principle Project Officer at Department of Health and Human Services Victoria and Jo Stevens Manager Chronic and Complex Care at Barwon Health will join us to discuss this innovative value-based approach being explored by the Victorian Department of Health and Human Services.

HealthLinks: Chronic Care aims to test the impacts of flexible funding models on the care outcomes for patients at high risk of multiple unplanned hospital admissions experiencing chronic and complex conditions. Health services have the flexibility to develop their own intervention model, using projected inpatient funding to design care around the needs of patients at high risk of multiple unplanned hospital admissions. This can include services that reach beyond traditional hospital-based settings, delivered by a range of providers. Some services have adopted a continuous service improvement approach and others are developing new models of care.

The department is conducting an evaluation of HealthLinks: Chronic Care in partnership with the Commonwealth Scientific and Industrial Research Organisation (CSIRO). The evaluation aims to examine whether offering flexible funding to health services enables them to develop and implement alternative models that provide better experiences and outcomes for patients with chronic and complex conditions at equal or lower cost.
