South Western Sydney PHN (SWSPHN) is a not-for-profit health organisation which is responsible for identifying and addressing the primary health needs in South Western Sydney through strategic planning, commissioning services, supporting general practices and other healthcare providers and supporting the integration of local healthcare services.

About the Role

Practice support is one of the core functions of the PHN. The role provides practice support to general practice and other primary health care providers in SWSPHN’s region to build health workforce capacity and the delivery of high quality health care for patients. A diverse role that involves a range of support activities including quality improvement and best practice methods, practice accreditation, professional and workforce development, digital health, patient data management and cultural awareness. If you have a passion for driving change, a willingness to learn and you are able to build strong relationships with internal and external stakeholders please apply.

Note: A current NSW driver’s licence and access to a comprehensively insured motor vehicle is required for this role.
