The Australian Healthcare and Hospitals Association (AHHA) welcomes the opportunity to provide feedback on the refresh of the National Strategic Framework for Chronic Conditions.
AHHA strongly believes there is a need for a whole of system approach to the prevention and treatment of chronic disease that is built around a shared vision for achieving the outcomes that matter to people and communities through the equitable, sustainable and transparent use of resources.
To achieve this, investment is needed in operationalising the broad range of policy commitments that impact the prevention and treatment of chronic disease in a coordinated, whole-of-system manner that supports grassroots innovation. The Deeble Institute for Health Policy Research Perspectives Brief ‘Transforming for value-based health care: Lessons from NHS Wales’ outlines a framework for driving value through identifying and supporting opportunities for optimisation at the macro (population health), meso (pathways of care) and micro (person-centred care) levels of the system. It is a framework that brings together stakeholders at a local level, but also connecting with national infrastructure and policy environment.
