In countries with fast aging population, the prevalence of dementia and frailty will increase, placing significant demands on health and social care costs.
Using patient-level data of both tertiary hospitals and primary-care policlinics within the National University Health System (NUHS) cluster among older adults with dementia or cognitive impairment (CI) (≥65 years), this project aims to:
- Define and target the frail and malnourished persons with dementia or CI using administrative data
- Understand the prevalence of frailty (measured by Hospital Frailty Risk scores (HFRS) and Clinical Frailty Scale (CFS)), comorbidities, malnutrition and its associated outcomes
- Measure the cluster-wide patient-level utilization and cost of healthcare services including both inpatient and outpatient visits
- Optimize the long-term care for the older adults with dementia or CI by comprehensive assessment during admission and collaboration with the community care
