In 2015, the National University Health System (NUHS) in Singapore pioneered the Value-Driven Outcomes (VDO) framework to enhance value-based healthcare by optimizing patient outcomes relative to costs. This framework focuses on optimizing patient health outcomes relative to the costs incurred, promoting both quality and affordability in healthcare delivery. With ageing and rising prevalence of chronic disease, we recognized the need to look at a more integrated perspective from episodic to patient level analysis which encompasses the entirety of a patient’s interactions with the healthcare system aiming to enhance holistic patient care.
In 2024, NUHS’ VDO Office implemented Cluster-Wide chronic condition projects such as Diabetes, Gout and Chronic Kidney Disease, leveraging data across NUHS Cluster including primary care services provided by National University Polyclinics (NUP) and tertiary care services from Alexandra Hospital (AH), Ng Teng Fong General Hospital (NTFGH), and National University Hospital (NUH). This holistic approach provides a clearer understanding of each patient’s overall health trajectory over time.
This initiative aggregates data from all patient interactions which includes inpatient admissions, day surgeries, outpatient visits and emergency encounters to construct a comprehensive view of each patient’s healthcare journey. By identifying patterns within this aggregated data, the project aims to pinpoint areas for clinical improvement, optimize patient outcomes and develop personalized care strategies that address individual needs. Additionally, analysing the cost-effectiveness of treatments across the continuum of care strives for sustainable healthcare practices without compromising quality. This initiative underscores NUHS’s commitment to advance value-based healthcare through innovative data-driven methodologies.
